I have installed DiskCheckup yesterday as at times my computer get slow, for example if i am playing a video, play back get dead slow for some time, then it start working as normal.

Based on the log, you have only monitored your drive for 1 day. This is not enough time to get an accurate trend. You need to look at how the values degrade over weeks or months. So I don't see anything in the screen shot that makes me think the drive is failing.

UPDATE : This hard disk stopped booting on 27th. I added new hard disk, able to copy all data to new HDD. After 2 days, it completely stopped responding.

We'll it is great that the software predicted something that actually came true, but my earlier comment still stands, to have any hope of predicting future failure dates, you need a larger sample to get a better trend. And even then it is hit and miss.
